Chester County Health Department Lyme Disease Survey

Residents of Chester County know that this area is a hotbed for ticks and that ticks can carry Lyme Disease and many other diseases! Ticks know no geographical boundaries, as they can be found in our most rural and urban areas!

This summer, the Chester County Health Department is conducting a community based survey to understand the knowledge and experience of county residents as it relates to Lyme Disease. The results of the survey will inform how the Health Department's retools its community education and more.

This survey is intended for those who live or work in Chester County. The questions in this survey are focused on your knowledge of Lyme disease and your personal experience with it. The Chester County Health Department will use the results of this survey to inform opportunities to educate our community about Lyme disease. Your participation is voluntary and your answers are completely anonymous. No personal identification information will be asked of you. It should take no more than 5 minutes to complete.
