Community members are invited to participate in the 2022 Regional Health Assessment

The Regional Health Assessment (RHA) Collaborative is gathering feedback from as many community members as possible to help best meet the health needs of our communities.

Community members who live, work, learn, or play in Linn, Benton, or Lincoln region are invited to provide their input by participating in the community health survey at: https://www.surveymonkey.com/r/LBL_RCHS.

The community health survey closes on June 3, 2022.

The RHA Collaborative is a partnership between the InterCommunity Health Network Coordinated Care Organization, Samaritan Health Services hospitals, Confederated Tribes of Siletz Indians, United Way of Linn, Benton & Lincoln Counties, and the local public health departments of Linn, Benton, and Lincoln counties.

The RHA Collaborative is committed to conducting a health assessment every three to five years to find out what affects people's physical, behavioral, and social well-being. The RHA Collaborative gathers input from a diverse set of community partners and communities to improve community health planning. Results of the survey will help to inform the regional community health improvement plan for the next 3 years.
