Kindergarten Students

5 doses DTaP vaccine
4 doses Polio vaccine
3 doses Hepatitis B vaccine
2 doses MMR vaccine
2 doses Varicella/Chicken Pox vaccine

Immunizations Required for Incoming
7th Grade Students

If your child is currently up to date on all immunizations he/she will need 2 vaccinations prior to beginning 7th Grade.
1 dose Tdap vaccine
1 dose Meningococcal vaccine

Immunizations Required for Incoming
12th Grade Students

If your child is currently up to date on all immunizations, he/she will need a second dose of the Meningococcal Vaccine prior to beginning 12th Grade.

(Note: If the first dose of the Meningococcal Vaccine was administered after the child's 16th birthday, only one dose is required).
