  • Increase appropriate referrals to CV specialists of patients with valvular disease, atrial fibrillation, and congestive heart failure as well as to prioritize care for patients eligible for life-saving treatment
  • Ensure patients with critical CV diseases are receiving guideline-directed medical therapy
