Dallas ISD students 12 or older who are fully vaccinated against COVID-19 are eligible for $50 incentive

DALLAS - With the safety and well-being of staff and students in mind, Dallas ISD is offering a one-time $50 vaccination incentive to students ages 12 and older who are fully vaccinated against COVID-19. The incentive will be distributed in the form of a gift card that can be used as cash.

To receive the one-time vaccination incentive parents/guardians can provide proof of vaccination for each of their eligible students by following these simple steps:

1. Scan the QR code on this card with the camera on your smartphone or visit www.dallasisd.org/studentvaxincentive to find the form.

2. Access the vaccination incentive form with the student ID number and date of birth and fill it out. Make sure you have access to the email address on the form.

3. Take a picture of your vaccination document (vaccination card, IMMTrac2 print out or written confirmation from a medical provider) and upload it.

Once parents submit the completed form, they will receive a confirmation email indicating that their form has been received and will be contacted with information about picking up the gift cards through that same email. The student's vaccination status will be used to process the vaccination incentive gift cards and to facilitate contact tracing and quarantine when required.

Nov. 15 is the last day to submit proof of full vaccination against COVID-19 and get their incentive gift card.

Estudiantes de Dallas ISD mayores de 12 años que estén vacunados contra COVID-19 pueden recibir incentivo de $50

DALLAS - Con la salud y el bienestar de los estudiantes y empleados en mente, Dallas ISD está ofreciendo un incentivo de vacunación de $50 disponible solo una vez para los estudiantes de 12 años de edad en adelante que estén completamente vacunados contra el COVID-19. El incentivo se distribuirá en forma de una tarjeta que se puede utilizar como si fuera efectivo.

Para recibir el incentivo de $50, proporciona un comprobante de que has recibido todas las dosis indicadas de la vacuna siguiendo estos pasos:

1. Escanea el código QR que se encuentra en esta tarjeta utilizando la cámara de tu teléfono o visita www.dallasisd.org/incentivoestudiantes.

2. Ingresa a la forma utilizando el número de identificación de estudiante y fecha de nacimiento para poder llenarla. Asegúrate de tener acceso a la dirección de correo electrónico en la forma.

3. Tome una foto del comprobante de vacunación (tarjeta de vacunación, copia impresa del IMMTrac2, o confirmación por escrito de un médico) y súbela al documento.

Una vez llenada y entregada la forma, las familias recibirán una confirmación por correo electrónico y a través de este mismo medio recibirán información de cómo recoger la tarjeta del incentivo. El estado de vacunación del estudiante se utilizará para procesar el incentivo de vacunación y facilitar el rastreo de contactos y cuarentena cuando se requiera.

El 15 de noviembre es la fecha límite para presentar el comprobante de vacunación contra el COVID-19 y recibir la tarjeta de incentivo.
