Hart Named Vice President of Long Term Care for Kaleida Health

Jonathan P. Hart has been named vice president of long-term care for Kaleida Health effective July 26, 2021.

In this role, Hart will oversee Kaleida Health's long-term care service line, which includes HighPointe on Michigan and DeGraff Rehabilitation and Skilled Nursing Facility.

A licensed nursing home administrator with more than 24 years of experience, Hart joins Kaleida Health from Our Lady of Peace in Lewiston, NY, where he served as their executive director. There, he led the 250-bed, multifaceted skilled nursing facility and improved their federal quality rating from 3-Stars to 5-Stars in less than 14 months. He also led the facility's COVID-19 effort and implemented systems to navigate the pandemic that were recognized as best practice on a national level.

Hart previously held other long-term care leadership roles, including executive director of The Amberleigh, a 250-bed independent housing and assisted living facility in Williamsville; administrator at Harris Hill Nursing Facility and Garden Gate Healthcare facility - both part of the McGuire Group; administrator/CEO of Catholic Health's System's Nazareth Skilled Nursing Facility; administrator of Episcopal Residential Health Care Facility in Buffalo; and administrator at Presbyterian Homes of Western New York. Hart is a veteran of the United States Air Force serving as a medical service corps officer. He was awarded the Army Accommodation Medal in 2005 for his service in support of the United States Central Command's 'Exercise Bright Star.'

Hart received his bachelor's degree from St. John Fisher College in Rochester and his Master of Public Administration, Health Care Management from SUNY Brockport. He also serves as a member of the Board of Directors of the Grand Island Catholic Club.
